Ordering Transcripts

Transcripts can be ordered by fax, mail or in person. We cannot accept telephone requests. If you have a VISA, MasterCard, Discover, or American Express card, you may pay for your transcript using this method. Your request will be processed within 48 hours.

Please note:  If you have a financial hold on your transcript, we will not be able to fulfill your request. If you suspect that this is the case, contact the Student Accounts Office at (309) 438-5643 before you send in a request.

Mail and Fax

  1. Download the Transcript Request Form (PDF 89KB).
  2. Fill out the form. Remember to check the appropriate credit card and include your credit card account number and expiration date.
  3. Sign the form. Federal regulations prevent us from processing unsigned requests.
  4. Fax your request to (309) 438-7023.

Mail

  4. Mail the form and payment to the address on the form


You may also request a transcript by writing a letter, including your signature, and either faxing it to (309) 438-7023 with the name of your credit card, your credit card number and expiration date or mailing it to Illinois State University, Transcript Services, Campus Box 2207, Normal, Illinois, 61790-2207.

Your letter must include the following information:
Name (include former names); your complete current address; including zip code; your University Identification Number; your date of birth, your last date of attendance; the complete mailing address, including the zip code, of the person or persons to whom you wish the transcript sent; a daytime phone number; a check for the appropriate amount or your credit card information (the fee is $6 per transcript); and your signature. Your request cannot be processed without your signature.

In Person

Transcripts ordered in person at the Student Service and Referral Center by 3 p.m. the previous day are available for pick-up the next business day after 11 a.m. Please note that transcripts are not available on demand. The Student Service and Referral Center is located in Moulton Hall room 107. You may fill out a request form there and pay by check, credit card, Redbird Card, or in cash. If you pay in cash, please bring the exact amount ($6 per transcript), as we have no provision for making change. Also be sure to bring a picture ID. Office hours are from 8am until 4:30 p.m. Monday through Friday.

OR

You may initiate a transcript request by coming to the Student Accounts Office at 607 West Dry Grove Street. The office is open from 8:30 a.m. until 4:30 p.m., Monday through Friday, except Thursdays the hours are 9 a.m. to 4:30 p.m. The cashier will need to see valid photo identification and will require payment of $6 for each transcript requested. You may also verify that your financial obligations have been met. Note: to pick up your transcript at the Student Service and Referral Center the next business day, the request must be made by noon the previous day.
